Keep Things Blameless

When deployments do go wrong—and they of course will at some point—blaming individuals is entirely pointless. Blaming individuals is the easy way out, allowing you as a team not to make any changes or harden any of your processes. Depending on the culture of your team, some team members may feel it is the noble thing for them to do to fall on their sword and accept the blame for a mistake which cost the company money or reputational damage. Don’t allow them to do this. Do the difficult work instead and consider what you can all do differently in terms of process.

Postmortems (or root cause analyses, or retrospectives, or whatever you might want to call them) are a great way to do this. Gather your team and talk through:1.

What happened?

Create a timeline together of the events that led to your incident and anything that happened in the aftermath of said incident. Include any notable and relevant decisions, interactions, failures, errors, and consequences, and include actions taken both by people inside the team and outside the team and any external factors that either contributed to the incident happening or exacerbated the impact of the incident.

This should give your team a great shared understanding of what happened, the factors that played a part, and make the rest of the exercise easier.
